Our Health and Safety Plan

Please review before your appointment!
Good Form’s top priority is the safety of our wonderful staff and clients. We will continue to strive to provide you with the absolute best experience that we can, and in order to serve these two important goals, we have made some changes.
Mask are still required in the procedure area
Due to the proximity required for piercing and tattooing procedures, all clients are required wear a mask during your procedure (with the exception of under-mask services). Your practitioner reserves the right to waive this mandate depending on circumstance. In the lobby and common areas of the shop masks are optional.
We have disposable masks available at all times.
Under mask piercings
As of December 2021, we have been offering under mask services again. Clients still need to wear a mask during any services that aren't under the mask.
No walk ins
Going forward we will be operating on an APPOINTMENT ONLY basis. All appointments must be booked in advance, this includes; consultations, tattoos, piercings, troubleshooting, jewellery changes, and jewellery sales.

Limited spectators
To minimize the number of people in our small space, we cannot currently allow more than one friend, family member, or support person to come with you to your appointment. We do not currently have the facilities to allow children who are not receiving piercings or services in the studio.
In the case of persons under 16 receiving a piercing, one parent or guardian will be required to come inside with them, and will be their one guest allowed.
If you are feeling unwell
DO NOT ENTER the studio if you are currently experiencing flu-like symptoms such as cough, fever, and shortness of breath. For the safety of all, please contact us to reschedule your appointment. You will not be penalized for rescheduling due to illness.
Eat before you arrive
To prevent lightheadedness, we require that you have eaten a meal within four hours of your appointment. As we do not allow the removal of masks while in the studio, you may not eat or drink during your appointment, but only outside during breaks.
Curb-side pickup
If you know exactly the jewellery you need, including; gauge, length/diameter, style, etc, and do not need it inserted, we can organize a contactless pick up at the studio that does not require you to enter the premises.
